ENROLL NOW FOR THE 2012-2013 SCHOOL YEAR
Student registration is now taking place for all students ages 2.5 and up at Lockland Elementary and Lockland District office.  Please stop in as soon as possible to register!  We want to have the right number of teachers for the students we will have attending this year. 

Enroll Monday-Friday in the District Office from 8-3 each day.  Enter the district office from the playground.  The door is the single glass door near the interal corner of the elementary and middle school. 

Items needed for Kindergarten student enrollment:
  • Student registration form
  • Birth Certificate
  • Immunization Records
  • Proof of Residency
  • Parent/guardian contact form
  • Student Health History
  • Dental Records
  • Physician's Report
  • Tuberculosis Screening Questionnaire
  • Social Security Card/ # of the student
  • Custody/Divorce Paperwork (if applicable)
  • Special Education or Medical Evaluations (if applicable)

Items needed for K-5 enrollment:
